If you received an invitation to complete the Phase Two online application, please click the Phase Two application button below and log in using the same User ID and password that you used to create your account. The Phase Two online application, along will all supporting documentation, must be submitted no later than February 25, 2014 (by 12:00 p.m., noon, Central Time). Hoewever, many faculty representatives will set an earlier campus deadline to allow ample time to review applications and determine their nominees. Please check with your faculty representative regarding the campus deadline.
Phase Two application requirements include:
Continuation of student online application, including transcripts
Student Financial Form, including a copy of 2012 Federal Tax Return
Parent Financial Form, including a copy of 2012 Federal Tax Return (required for applicants under 30 years of age)
Second online recommendation
Nomination by the Faculty Representative at your undergraduate institution (due February 27, 2014)
The Phase Two deadline is February 25, 2014 (closes at 12:00 p.m., noon, Central Time).
Please be aware that many institutions have very strict regulations on their email systems and the email from our application to your recommender or parents may be mistaken for spam. PLEASE ensure that your recommender and parent have received the email and, if not, Contact Us as soon as possible.

COMPLETE THE PHASE TWO APPLICATION
You may complete the application in any order you prefer. Applicants may return to the application to update and edit as frequently as desired until you submit your application or the application deadline has passed.
To avoid losing data that you enter:
Do not use your browser’s "Forward" or "Back" button
Frequently click the "Save" button
Always use the "Logout" link on the navigation bar (located on the left side) to exit
The required online recommendation form must be submitted by an arts or academic professional (an advisor, instructor, or professor well acquainted with your work). This person should be an arts or academic professional at your current or most recent institution.
The recommendation form must be submitted by your recommender by 12:00 p.m., noon, Central Time, on February 25, 2014. To ensure that your recommender receives the email notification in time to submit the recommendation, you should complete that section of the application no later than February 21, 2014.
It is the applicant’s responsibility to confirm that the recommender received the email link to the recommendation form.
