Renewal Forms

As we begin the new calendar year, the Foundation will conduct part one of the Young Scholar renewal process and will review information about each Young Scholar to ensure that he or she may continue to participate in the program in 2010-11. In part one of the renewal, we will review the Young Scholar’s academic performance, participation in activities, and conduct, as well as the family’s cooperation with the Young Scholars Program. 

 

Please note that each Young Scholar must complete a renewal form at this time. In addition, one parent/guardian must complete a renewal form for each Scholar.

 

If a Young Scholar will apply to college during the fall of 2010, we also gather information that will help us assist you during the college application process.

 

 

Thank you for using the Foundation website to access your renewal information. After you read this information, please click on the appropriate document(s) in the orange text to the right.

·                 If you are a Scholar, please click on the Scholar Renewal Form and complete it.

·                 If you are a Scholar who will apply to college during fall 2010, also complete and return the Scholar College Questionnaire as part of your renewal process.

 

·                 If you are a Parent/Guardian, please click on the Parent/Guardian Renewal Form and complete it.

 

Once you click on a document, you can type your responses directly into the form. Remember to save the file on your computer. After you have completed and saved the forms, please email them as attachments to Lauren Grossman at lgrossman@jkcf.org.

 

If you prefer that we send a paper copy of the form(s) to you, please email Lauren Grossman (lgrossman@jkcf.org) or call her at 703.723.8000, extension 291. We will gladly mail forms for you to complete and return to us by mail.

 

In the spring, the Foundation will conduct part two of the renewal process by reviewing each family’s financial information. Please expect to receive the financial forms in March; these financial forms, along with a copy of your 2009 tax returns, will be due to the Foundation in mid April.
