Renewal Forms
This link is to be used by current Young Scholars and their Parents/Guardians to complete the annual renewal process.
Dear Young Scholar and Parent/Guardian:
To access your Renewal Forms, click on the appropriate document(s) in the orange text to the right. Please carefully review and complete the 2010 Letter of Agreement (a Spanish translation of the Letter of Agreement is also available) and the appropriate Financial Renewal Form(s). In order to determine which financial form(s) applies to your family, use the chart below.

Complete Financial Form A
|
if the Scholar's parents/guardians are married to each other.

Complete Financial Form B
|
if the Scholar's parent/guardian is single, divorced, separated, or widowed
OR
if the Scholar's custodial parent/guardian is married to someone other than the Scholar's other biological or adoptive parent.

If you are completing Financial Form B1, please forward Form B2 to the Scholar’s biological or adoptive father.
If you are completing Financial Form B2, please forward Form B1 to the Scholar’s biological or adoptive mother.
Once you have completed the relevant form(s), please return the following:
· 2010 Letter of Agreement, signed by the Scholar and at least one parent/guardian
· 2009-10 Scholar Financial Renewal (all forms that pertain to your family situation)
· 2009 tax returns, including all W-2s, schedules and attachments that you send to the IRS
· Supplemental Self-Employment Form, if you are a business owner
